Using the Enrollment Form Template
How to build your registration form
Enter your organization name and form title, add your logo, and tick the sections you want. List your programs to turn them into checkboxes, and list the policies families must initial. The preview updates as you go; print it or download a PDF when it is ready.
Which sections to include
At a minimum, collect student details, parent or guardian contacts, emergency contacts and medical or allergy information, because staff need them on day one. Add authorized pickup for children, program selection when families choose classes on the form, and policy acknowledgment so families confirm they have read your tuition, refund and attendance rules.
Keep payment details off paper
A paper form is a poor place to write full card or bank account numbers. The payment section asks families to choose a payment method and authorize billing, and tells them you will collect payment details through a secure payment system instead.
Common mistakes
Asking for information you never use makes forms longer and less likely to be completed. Missing allergy or authorized-pickup details cause real problems later. Review the form at the start of each term and remove anything you no longer need.
